Step 1: Intake Interview

  • Call 973-247-8988; call or text 862-287-2688 or complete Client Intake Form in Contact Us section 
  • As part of Intake process, we will ask you questions on your medical history and current problems to establish the relevant information to proceed.
  • Please be aware that at Step 1, you may not necessarily be talking with the Director of Nursing.

Step 2: Nursing Assessment

  • Your Nursing Assessment is carried out by RN to develop Care Plan.
  • Please be aware that you may find the Nursing Assessment long and narrative.
  • It is deliberate nursing process to understand your care needs and elicit any health risks.
  • In-Home Nursing Assessment is free, if we complete service agreement. 
  • For the Nursing Assessment, we encourage the client and a member of the family to be present.

Step 3: Care Plan & Caregiver Compatibility

  • Your Care Plan is customized based on RN assessment and your direct inputs.
  • Your Care Plan is your care needs and tasks  to be performed.  
  • Your care needs is deliberately matched with Caregiver competence. 
  • ECHO Caregivers are multidisciplinary, multitasking, physically strong and mentally prepared.
  • Client and caregiver can "Meet  & Greet" before Start of Care
  • Your Caregiver is supervised.
  • Your Care Plan is reviewed regularly to meet your care needs

Step 4: Start of Care [SOC]

  • Caregiver completes orientation on your personalized care plan prior to SOC
  • A copy of Care Plan containing all the scheduled tasks of the Caregiver is kept with you
  • You will sign off on the daily / weekly duties performed by Caregiver.
  • We conduct supervisory visits to ensure that you are receiving  effective care. 
  • You will evaluate the Caregiver and our Services using our 5-point grade system. 
  • We are committed to provide quality care.
